What happens at the appointment?
We will talk about your diet, lifestyle, and questions. Together we will set up a plan to help you reach your goals.

- Nutrition Counseling
- Taking Care with Diabetes is a diabetes self-management education program led by a certified instructor. The program includes individual counseling appointments and group workshops.

How much will it cost?
We accept Medicaid, Medicare, Blue Cross Blue Shield, Aetna, and other insurance plans that include nutrition counseling as a covered benefit. For those not covered by insurance, fees are based on a sliding scale according to income.
How I make an appointment?
Please fill out and submit a referral form here. Our staff will call you to set up an appointment.
Interpretation for non-English speakers and people with hearing impairment is available. Home visits are available for children and those unable to come into the clinic.
